Jade Vergara graduated with her MSN in 2013 from VUSN. Her clinical experience is in home based primary care services for older adults in the community setting. She returned to VUSN in 2019 to teach in the Pre-Specialty Community Health coursework.

Caring Across Generations: Strategies for Building Age-Friendly Health Systems
This workshop will introduce participants to integration of evidence-based and evidence-informed age-friendly care practices into health systems. Participants will develop an action plan for preparation and implementation of the Institute for Healthcare Improvement's 4Ms Framework for age-friendly care in their own clinical settings.
